Dr. Haltom and staff welcome you to Glenwood Oral Surgery, P.C. Our policy is to openly discuss our diagnosis and treatment recommendations with you. We encourage you to ask any questions that you may have.
We encourage a discussion of fees and billing procedures and prefer to answer any questions or concerns at the beginning of treatment. Payment in full is due when services are rendered unless specific arrangements are made in advance. We accept Visa, MasterCard, American Express, Discover, and CareCredit.
Pre-authorization for insurance benefits is recommended for all elective treatment. This will inform you in advance how much your insurance will pay for your proposed treatment. The difference between your insurance benefit and your treatment fee will be the amount you are expected to pay at the time of service.
As a courtesy, this office will file your insurance for you, but payment of your account is your direct responsibility. This office does not take responsibility for negotiating disputed claims. Questions should be directed to your insurance company rather than this office.
If you have insurance benefits and no pre-authorization is done, the expected payment will be 50% of the treatment fee with insurance payment assignment directed to our office.
All balances are immediately due within 90 days of treatment, regardless of insurance payment received. If your insurance requires that payment is sent directly to you, or if there are no insurance benefits available, full payment is required at the time of service.
